Advances in Newer Arypical Antidepressants

Abstract
There have been a close relationship between the updating of biochemical theories of depression and the development of antidepressants. The monoaminc theory of depression was postulated after the introduction of tricyclic antidepressants (TCAs) and monoamine oxidase inhibitors (MAOIs) in clinical practice. The monoamine theory was founded on the fact that the TCAs block the reuptake of the serotonin and the noradrenaline in the synaptic clefts. The effects of TCAs on other receptor systems were thought to be related with side effects. Thus, the selective serotonin reuptake inhibitors (SSRIs), which have solely effects on one neurotransmitter system, was developed. Since the development of SSRI, serotonin system has been the focus of the depression study. The SSRIs¡¯ limited efficacy on severe depression called for the develop-ment of newer antidepressants which have both the efficacy of TCA and the compliance of SSRI. These newly developed antidepressants provide some new perspectives on the pathophysiology of depression. In this article, we will review the mechanism of action, efficacy, the side effect profiles of newer atypical antidepre-ssants, and the implications of their mechanisms of action in the pathophysiology of depression.
